Job Description

Dakota Center for Independent Living, Inc. (DCIL) has an immediate, full-time (40 hours/week) job opening for a Money Follows the Person (MFP) Transition Coordinator. This position will be responsible for all aspects of transitioning individuals from a higher level of care (i.e. skilled nursing facilities, hospitals, ICF/ID group homes, etc.) into the community of their choice. The person applying for this position must have strong planning, communication, organizational, and interpersonal skills, ability to write detailed notes, work well as a team member or independently, be outgoing, motivated, and self-directed. Occasional work beyond the regular schedule may be required. Must upload a resume to be considered for this position.

SUPERVISORY RELATIONSHIPS
Reports to the MFP/TDP Program Director.
SUPERVISORY AUTHORITY
No supervisory authority.
ESSENTIAL JOB FUNCTIONS 1.
Become familiar with all aspects of the center's programs, services, the philosophy of independent living, and history. 2. Determine eligibility, secure the pertinent background information and complete the center's intake packet on all new referrals. 3. Establish and maintain consumer service records, record information and referrals, provide monthly, quarterly and annual reports as requested by the Program Director. 4. Establish and maintain a cooperative working relationship with consumers, outside agencies, and nursing facilities involved in the transition process. 5. Provide DCIL and MFP information to consumers as required in the MFP Operational Protocol. 6. Assess involvement of consumer's guardian, provide Guardianship expectations information, and document activities in the consumer's records in the agency data management system and MFP data management system. 7. Conduct MFP transition assessment of consumer needs, resources, and support, assist the consumer with Transition Assessment, developing an Transition Plan, develop A Risk Assessment and Safety Plan, develop a 24-hour backup plan, and document consumer notes in CIL First and Therap. 8. Assist the consumer and transition planning team in identifying support needs and services. Ensure resources the consumer will need (housing, transportation, medications, personal assistance, etc.) are in place before the transition from the nursing facility to the community. 9. Provide follow-up to consumers in their homes for 365 days post-transition. 10. Attend all in-services, training, and meetings as requested by the MFP Grant Program Administrator. 11. Must be able to travel within service delivery area frequently. 12. Other duties as assigned by Program Director/Executive Director.
OTHER JOB FUNCTIONS 1.
Participate in scheduled supervisory/staff meetings. 2. Participate in-service and professional development training. 3. Maintain consumer confidentiality. 4. Comply with all DCIL policies and procedures. 5. Possess knowledge of community services and resources within the center's service delivery area. 6. Participate in an annual performance evaluation. 7. Participate in the planning and development of the center's annual year plan. 8. Have a working knowledge of Microsoft Office.
SPECIAL CONSIDERATIONS
The Money Follows the Person Transition Coordinator must know about physical, social, and psychiatric disabilities, evaluate daily living skills, and collaborate with consumers to identify interests, determine Independent Living potential, and set appropriate goals. The person filling this position must coordinate services and appropriate follow-up service plans after the consumer achieves their goals.
QUALIFICATIONS 1.
Experience in the human services field is preferred. 2. Exceptional interpersonal communication skills. 3. Experience working with individuals with disabilities. 4. Experience collaborating with local, state, and federal agencies and organizations.

Duties will be performed in a variety of settings such as in the office, community, and consumer homes, and throughout the service area. 5. Required to talk, hear, and have specific vision abilities, such as close vision, distance vision, and ability to adjust focus. 6. Must be able to lift or transfer a minimum of 20 pounds. 7. Frequently required to sit, stand, walk, reach with arms and hands, climb/balance, stoop, kneel or crouch. 8. Travel is required. Must have reliable transportation and be able to travel in our 18 county service delivery area. 9. Occasional work outside of core business hours will be required. 10. The position is open until filled. Pay is based on education and experience.
EDUCATION
High school graduation, or its equivalent, and any equivalent combination of training and experience that provides the required knowledge, skills, and abilities.
