Overview Make a meaningful impact in community health as a Health Services Coordinator. In this dynamic role, you will serve as a vital link between healthcare providers, patients, and community resources to ensure seamless access to quality health services. Job Summary Eckert Youth Homes is seeking a Health Care Coordinator to oversee medical coordination for youth in our residential treatment program. This role focuses on organization, communication, and follow-through, not clinical care. This position is non-nursing and does not include licensed nursing roles (RN, LPN, CNA, CMA). Key Responsibilities Coordinate and schedule medical, dental, and specialty appointments Track provider recommendations and follow-up care Manage medication ordering, refills, and communication Maintain organized and compliant documentation Communicate updates with staff, providers, and families Support intake, discharge, and team coordination Provide staff training on basic medical-related procedures Transport youth to appointments as needed Qualifications Bachelor's degree in health-related or human services field preferred Experience in healthcare coordination, behavioral health, or residential care preferred Strong organization, communication, and multitasking skills Knowledge of HIPAA and trauma-informed care preferred Valid North Dakota driver's license required Must be at least 21 years of age Position Details Full-time (40+ hours/week) Evening, weekend, and on-call rotation required Join us as a Health Services Coordinator and be part of a dedicated team committed to fostering healthier communities through compassionate coordination and proactive engagement.
