Patient Care Coordinator

We're Hiring! Join our team as a Patient Care Coordinator with a focus on Chronic Disease Management. Are you passionate about helping patients take control of their health and improve their quality of life? We're looking for a dedicated Patient Care Coordinator to join our Valley County Health System (VCHS) team. This is full-time position working 5 days per week. In this role, you will: Provide education and support to patients living with chronic conditions such as diabetes, heart disease, COPD, and hypertension Develop individualized care plans Monitor patient progress and outcomes Collaborate with interdisciplinary healthcare teams Empower patients to build sustainable, healthy habits Provide direct patient care within the medical clinic The ideal candidate is compassionate, organized, able to be a self-starter, can work independently, and is committed to patient-centered care. Experience in care coordination, patient education, and chronic condition management is preferred. Applicants must have current Nebraska licensure as a Registered Nurse.
