Job Description

Responsible for ensuring that the primary operations of the case management department function efficiently. Maintains a caseload or assignment as per requirement while providing a first line resource to social workers. Acts as a liaison for medial and social issues that may arise, and communicates information and recommendations to the management as needed.
Principal Accountabilities:
1. Assesses the phyco-social needs of the patient and family and develops a treatment/care plan in concert in with patient, physicians, nurses and other members of the departmental team. 2. Provides crisis intervention, and supportive counseling to patients and their families as needed and may refer to outpatient resources as appropriate. 3. Serves as patient advocate and liaison with physicians, families, insurance company, community agencies and others as needed to ensure continuity of care. 4. Links patients/family to the appropriate community support groups and resources including, information/referral to sources of financial assistance, transportation, support groups, and other community services. 5. Documents patient/family status, diagnosis, treatment plan, goals, and interventions, evaluation results, observations and progress in medical records. 6. Assists with the interviewing, orientation, training and ongoing development of program staff including coordinating team meetings and managing staff coverage. 7. Provides clinical and administrative supervision for social workers and clinical social workers on the team. 8. Provides discharge planning based on meeting identified goals of the treatment plan anywhere. 9. Performs other related duties as assigned.
Required:
1. MSW (Master of Social Work) Degree from an accredited school of social work or Master's Degree (or higher) level clinician in Psychology, Social Work, Counseling or Marriage or Family Counseling, or Licensed Ph.D. in a related field. 2. LCSW (Licensed Clinical Social Worker). 3. 2+ years of experience in healthcare or mental health care or a related field.
Preferred:
1. Four (4) or more years of experience in healthcare or mental health care or a related field with managerial/supervisory. 2. Two (2) or more years of case management experience in either inpatient or outpatient setting with knowledge base of discharge planning, community health, managed care, Medicare, Medicaid requirements etc.
