Job Description

Patient Care Coordinator & Scheduler At CNSO, we believe exceptional care begins the moment a patient reaches out for help. Our Care Coordinators and Patient Schedulers play a vital role in helping individuals access the specialized care they need experiencing compassion and professionalism along the way. Our specialties include neurosurgery, orthopedic spine surgery, pain management, reconstructive surgery, plastics, and physical therapy. Position Summary We're looking for a friendly, confident communicator who understands that every interaction shapes a patient's experience. The ideal candidate thrives in a busy office, can manage multiple priorities at once, and enjoys helping patients navigate their care from the first phone call through follow-up. This role combines scheduling, administrative support, and Patient Access & Care Coordination style coordination to ensure patients receive timely, organized, and compassionate care.
Responsibilities:
As a Patient Care Coordinator, you will help optimize the daily flow of physician schedules while supporting patients who depend on us for timely, coordinated care. This role requires strong multitasking skills, sound judgment, and the ability to manage a high volume of patient needs in a fast-paced specialty practice.
You will:
  • Serve as a primary point of contact for patients by phone and email, delivering an exceptional customer service experience
  • Schedule clinical appointments accurately and efficiently, balancing provider availability, urgency, and patient needs
  • Respond to provider requests to arrange follow-up care for hospitalized patients in the appropriate office location
  • Enter and update demographic, insurance, referral, and scheduling information in the electronic medical record (EMR) system with a high level of accuracy
  • Assist with scanning, faxing, and other administrative tasks that support smooth daily operations
  • Communicate clearly with patients, providers, hospitals, and team members to ensure timely follow-through on scheduling and care coordination needs
  • Act in a patient care coordinator support capacity by tracking patient follow-up needs, coordinating referrals, obtaining and organizing outside records, and helping patients move through the next steps of care
  • Monitor outstanding tasks, messages, and patient requests to ensure issues are addressed promptly and nothing falls through the cracks
  • Help guide patients through complex scheduling, referral, and insurance-related processes with professionalism, patience, and empathy
  • Prioritize urgent matters while managing multiple phone calls, emails, provider requests, and patient concerns throughout the day
  • Coordinate communication between patients, providers, hospitals, imaging centers, and ancillary departments to support continuity of care
  • Escalate clinical or scheduling concerns appropriately and collaborate with the care team to support safe and efficient patient follow-up
  • Assist patients in understanding appointment requirements, referral needs, and documentation needed to avoid delays in care
  • Support physicians and office staff by maintaining organized workflows and helping manage the demands of a busy multi-specialty practice Qualifications
  • Enjoys being busy and thrives in a high-volume, fast-paced office environment
  • Comfortable with frequent patient interaction and able to guide patients through complex scheduling, referral, and insurance-related needs
  • Strong attention to detail, especially with demographic, insurance, and referral data entry
  • Prior experience with insurance verification, authorizations, or referral coordination is a plus
  • EMR experience required (eClinicalWorks preferred)
  • Strong written and verbal communication skills
  • Bilingual abilities are a plus
  • Excellent multitasking, organizational, and follow-up skills
  • Ability to remain professional, compassionate, and efficient under pressure
