Position Summary The Home Care Patient Coordinator will play a vital role in maintaining and expanding our patient census while ensuring patients receive timely, compassionate, and properly staffed care. You'll be the critical link between clients, caregivers, and internal operations. This is not a typical 9-5 desk job. We're looking for someone who is career-oriented, proactive, and committed to growing with us long term. Responsibilities Coordinate and schedule care between patients and caregivers Ensure all cases are properly staffed and restaffed as needed Assist with payroll and pre-billing processes Promote and monitor EVV (Electronic Visit Verification) compliance Monitor case retention and step in to support where needed Ensure high levels of client satisfaction through follow-up and support Collaborate with field staff, office team, and management on daily operations What We're Looking For Prior experience in home care, healthcare staffing, or case coordination Strong communication and organizational skills Ability to work in a fast-paced, detail-driven environment Familiarity with EVV systems, EMR, and billing processes is a plus Positive attitude, problem-solving mindset, and team-first approach Someone who's career-focused and eager to grow with an expanding company Why Join Stand Out Home Care?
