Job Description
Partners for Kids and Families , a nonprofit agency, is in search of a Care Manager. The Care Manager works with families and youth (ages 5-21) who are experiencing a variety of behavioral/emotional challenges. The youth may have a mental health diagnosis, an intellectual/developmental disability, or substance use disorder. Using a wraparound approach, the Care Manager facilitates a Child-Family Team to provide the family with supports and services. The applicant must have a Bachelor?s degree and prior experience working within the social service field assisting children and families. The candidate should understand the importance of cultural competency, be proficient with computers, be flexible and able to work evenings, have excellent communication skills, be creative, and have an enthusiasm for helping others. Spanish speaking candidates encouraged to apply. The Care Manager is the primary representative of Partners for Kids and Families to the youth, families, natural supports, and providers. The Care Manager submits for authorization, coordinates, and monitors all services for the child and family. The Care Manager upholds the agency?s commitment to provide services and supports that are family-centered, community-based, strength-based, individualized, culturally competent, and easily accessible
The Care Manager:
Practices care management in accordance with wraparound values Develops and facilitates child and family team meetings Develops, documents, and distributes Individualized Service Plans (ISP) as developed by the Child Family Team (CFT) Completes progress notes as well as other documentation in accordance with agency time lines. Refers child and family to community resources Strictly enforces confidentiality regulations and HIPAA at all times. Adequately informs service providers of child and family?s history, reason for referral, services requested,service authorization(s), anticipated outcome(s), and requested progress reports Monitors ISP to assess child and family?s progress with respect to the stated family vision, needs, strategies, and outcomes Completes in-person, face to face visits with the youth and family as well as needed visits with relatives, providers, and other community resources Maintains regular and ongoing communication with child-family team members and agency supervisor(s) to gauge progress Obtains written progress reports from team members and system partners. Revises the ISP with the child-family team as needed Develops comprehensive crisis plans with the child-family team Participates as a member of the agency?s after hours on call rotation Responds to crises that occur with the child and family, including on-site intervention to telephonic crisis management and stabilization. Identifies community resources to meet the needs of the child and family. Participates in ongoing training sponsored by the agency or the Division of Children?s System of Care to facilitate professional development. Educates families and the community about wraparound values and the role of the Care Management Organization within the system of care Coordinates transition of child and family from the CMO and links them to any needed services/supports Develops positive relationships with internal and external resources. Other duties as assigned by theCare Manager Supervisor Salary Range:
$47,000.00 ? $48,000.00 per year, depending on experience and qualifications.Benefits and Compensation:

Two years experience providing direct care to children and families. Excellent communication, facilitation, assessment and system negotiation skills. Creativity and enthusiasm essential. Profeciency in the use of agency technology including, but not limited to Office 365 and videoconferencing platforms. Ability to read and comprehend documents such as policy and procedure manuals, agency specific documentation, and/or documentation from systems partners. Ability to write documentation capturing the activity of the Child-Family Team through progress notes and the ISP. Ability to write routine correspondence. Ability to speak effectively before groups of people or employees of the organization.Reasoning skills:
Ability to solve practical problems. Ability to interpret written and verbal instructions Driving skills: Ability to drive a car, potentially for multiple hours at a time.Mathematical skills:
Ability to add, subtract, multiply, and divide.Computer skills:
Proficiency in use of keyboard, mouse, computer.Physical demands:
The employee must occasionally lift and/or move up to 10 pounds. While performing the duties of the job, the employee is regularly required to stand, sit, walk, talk, and/or hear. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.Work environment:
