Full Job Description Job Title:
Discharge Coordinator Responsibilities:
  • Schedule patients for follow-up appointments and procedures, including making sure schedules are full
  • Entering and routing orders and referrals appropriately, including knowing where patients can go according to their insurance plan
  • Providing patients with applicable education materials, instructions, etc.
Qualifications:
  • Certification in Medical Assistant
  • At least 1 year of medical assisting experience
  • Time management skills
  • Ability to work well in a fast paced environment while multitasking
