We are an Oncology Medical Home, with a focus on early intervention for patient symptoms, resulting in low hospitalization rates. Our Physicians have won Top 10 Docs in the city of Albuquerque multiple times. As a successful Medical oncology and Radiation oncology practice, we have expanded our medical practice to include Survivorship, Palliative Care, Breast Surgery, Rheumatology, Pulmonary, Urology and Neurology. POSITION SUMMARY
The Patient Care Coordinator is a reflection of NMOHC's philosophy and dedication to superior healthcare and patient interaction and reinforces and adheres to the core values of the company in order to achieve the mission and vision. This position must demonstrate sensitivity and professionalism while assisting patients with scheduling and follow-up needs.
ESSENTIAL FUNCTIONS

Essential Functions Statement(s): Schedules, cancels, or reschedules all Radiology exams. Works with other PCC's via Microsoft Teams to schedule and coordinate exams with physician follow-up appointments. Ability to multi-task through various programs to schedule, including checking in patients and answering phones. Contacts patients regarding missed appointment and follows up on no show appointments. Entering disc requests for imaging that needs to be sent to outside facilities or for patient pick up. Answer and send emails to billing and other emails related to patient care. Sending out an auth pending list for next day and also for next two weeks Check e-fax Sending therapy list to techs Schedules labs and radiology exams for NMCC patients as well as outside referral patients. Go through and assist with faxes received Initiates prior authorization email to the billing dept. as needed for all diagnostic testing and follows up to make sure authorization has been received prior to patient exam. Monitors and reviews the radiology schedules on a daily basis, ensuring processes are followed and scheduling errors are corrected PRIOR to the patient arriving for the visit. Communicates openly and effectively with the physicians and professional staff. Ensures that confirmation calls are done to remind patients of their appointment, including the review of patient prep for scheduled exams. Answers all phone lines in the POD, tasks, and emails promptly and professionally. Assists with chart prep (this includes printing labs, making sure instructions are given, writing follow up dates, making sure all paperwork is done for patient to fill out, checking auth) Communicating with triage nurses for lab work that needs to be addressed and if orders need to be changed

SKILLS & ABILITIES
Education and Experience:
High School Graduate or General Education Degree (GED) Two to four years related experience preferred, preferably familiarity with Radiology procedures such as CT, PET, MRI and X-ray Minimum of two years' experience as a Patient Care Coordinator or Medical Assistant Radiology, Medical Oncology, and Radiology Oncology work experience is a plus
Computer Skills:
Intermediate computer skills Experience with OncoEMR and
PACS/RIS
is a plus
