For over 40 years, La Clinica has provided services to the residents of Southern New Mexico. Job Summary:
Under the supervision of the CEO, the Director of Population Health is a key leader in La Clinica de Familia's (LCDF) movement towards an outcomes-focused, team-based care model whose goal is to provide improved health equity, quality, and cost. The Director of Population Health drives strategic oversight of population health programs, including Chronic Care Management, Certified Community Behavioral Health Clinic (CCBHC) Care Coordination, and Community Health Worker (CHW) initiatives, to deliver high-quality, patient-centered care, advance value-based performance, and ensure compliance with regulatory requirements.

Exceptional communication skills Capacity to work effectively in a team structure Ability to interact respectfully with a diverse group of stakeholders of varying disciplines and educational levels Knowledge/skills in quality improvement Knowledge of medicine, public health, and health services issues Working knowledge of standard desktop computing software packages (word processing, spread sheeting, presentation software, Internet browsers, etc.) Familiarity with health care delivery in community health settings (preferred but not required)
Job Requirements:
Bachelor's degree in Nursing; Master's degree preferred A minimum of 2 years of Clinical Health outcome experience preferred Previous management experience preferred
