Clinical Supervisor- Per Diem

Post-Hospital Visit:

$35-$60 per visit ( Supervisory visits are typically brief (approximately 15-20 minutes) and include required documentation.

Reassessment:

$75 per visit

Initial Admission:

$90 weekday | $125 weekend

Schedule:

2 days a week. 8a-5p You will... Performs on-site supervisory and teaching visits to evaluate caregivers Provides clinical orientation to caregivers on assigned home care cases. Participates in the training of new nurses and in-services for field staff. Performs initial nursing assessments Counsels patient/caregiver in meeting nursing and related needs. Assigns and coordinates services provided. Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs. Prepares clinical progress notes ensuring any changes in the patient's condition is reported Monitors clinical records for complete and accurate documentation. May include instruction to caregivers and performing skills competency checks.
