Job Summary Responsible for the supervision and review of intake referrals to Agency services. Oversees the collaborative practice with management and staff to utilize proper clinical services and promote excellent patient outcomes. Responsible for the supervision of the intake department and staff. Responsibilities Review referrals to make sure the agency has the skills and resources needed to provide safe patient care. Coordinate complex patient discharges and work with clinical and intake staff to assess agency capacity and insurance coverage. Identify and resolve problems to support smooth operations. Improve intake processes and communication to increase efficiency and responsiveness. Verify insurance coverage and obtain prior authorizations when needed. Confirm the correct physician has approved home care services. Ensure required Face-to-Face documentation is accurate and complete; educate referral sources as needed. Supervise intake clinical and administrative staff. Respond to inquiries about agency services. Maintain and update the Intake Department Procedures Manual. Track and report referral activity and referral sources. Build and maintain positive relationships with hospitals and other referral sources. Assist with onboarding and training of new staff, students, and other learners. Inform leadership of referral source concerns and opportunities for education and relationship development. Represent the agency professionally at all times. Participate in agency committees as assigned. Maintain professional knowledge through continuing education and training. Provide holiday and weekend coverage when required. Ensure compliance with agency policies and all state and federal regulations. Perform other duties as assigned. Qualifications Valid NYS RN License 2-4 Years CHHA experience 2-4 years of Supervisory / Management experience 2-4 years of recent Medical Surgical or Acute Care experience

Pay:

$80,000.00 - $85,000.00 per year
