  • Full-time About Us We are a rapidly growing physician practice specializing in post-acute and long-term care services throughout skilled nursing facilities (SNFs), assisted living facilities (ALFs), and other healthcare settings.
Our mission is to improve patient outcomes, provide exceptional family support, reduce avoidable hospitalizations, and ensure seamless continuity of care across the healthcare continuum. We are seeking a highly motivated, relationship-oriented professional to serve as the primary liaison between our medical practice, patients, families, assisted living communities, and skilled nursing facilities. Position Summary The Patient & Family Liaison / Post-Acute Care Account Manager serves as the face of our practice in the field. This individual will build strong relationships with patients, families, ALF leadership, SNF administrators, Directors of Nursing, hospital discharge planners, and community partners. The ideal candidate is a proactive problem-solver who thrives on helping families navigate complex healthcare situations while ensuring an exceptional experience for our facility partners. This role combines patient advocacy, account management, business development, and care coordination. Key ResponsibilitiesPatient & Family Support Regularly visit assigned assisted living and skilled nursing facilities. Introduce our services to patients and families and ensure they understand how to access our team. Serve as a dedicated point of contact for families when communication gaps arise or additional support is needed. Maintain ongoing relationships with families and provide proactive outreach. Respond quickly to patient and family concerns and coordinate solutions with providers and facility staff. Ensure every family knows our practice is available as a trusted resource throughout the patient's healthcare journey. Hospital & Care Transition Navigation Monitor patient hospitalizations and care transitions. Assist families in navigating healthcare decisions during hospital admissions and discharges. Coordinate with hospitals, case managers, social workers, and facility staff to ensure continuity of care. Help facilitate transitions to preferred healthcare partners when clinically appropriate. Support families throughout the discharge planning process. Ensure smooth communication between hospitals, skilled nursing facilities, assisted living facilities, providers, and family members. Facility Relationship Management Conduct routine rounds throughout assigned SNFs and ALFs. Build strong working relationships with Administrators, Directors of Nursing, Social Workers, and other key stakeholders. Serve as the primary account manager for assigned facilities. Gather feedback regarding provider performance, responsiveness, and service quality. Identify and escalate operational issues to leadership. Ensure facility partners receive exceptional service and support. Business Development & Census Growth Identify opportunities to strengthen relationships with referral sources. Assist with patient placement and care transition opportunities. Collaborate with facility teams to identify residents who may benefit from our services. Work closely with leadership to support census growth initiatives. Identify opportunities for continuity of care within our network of healthcare partners. Help connect appropriate patients and families with available care resources. Qualifications Healthcare, senior living, post-acute care, assisted living, skilled nursing, hospice, home health, or healthcare sales experience preferred. Exceptional interpersonal and communication skills. Strong relationship-building abilities. Ability to work independently and manage multiple priorities. Comfortable spending significant time in the field visiting facilities. Experience working with families during healthcare transitions is highly desirable. Strong organizational and follow-up skills. Bachelor's degree preferred but not required for candidates with relevant experience.
