Job Description

Are you looking to empower residents, advocate for their families, and give our nursing staff the vital clerical support they need to succeed? Join our compassionate team as a Medical Coordinator and make a real impact on the lives of people we serve! We offer paid training, great benefits, and a supportive, mission-driven culture. As a Medical Coordinator, you'll support people in reaching their personal goals, making choices, and living independently as possible. You'll help with daily activities, promote community involvement, and ensure a safe, healthy, and clean environment. What you'll do: Coordinate referrals to community based medical specialty providers Assists with scheduling and attending appointments and procedures with persons served as needed Assists in delegated nursing care as directed by RN Ensures there is sufficient stock of medications, and the medication room is organized and clean Communicates with clinical staff to ensure follow up to medical appointments in their clinical area.
What you bring:
