Job Description

As a population health coordinator, you'll have an important role in preventive care and in addressing social drivers of health, and will work with patients toward improving health outcomes. Plus, you won't be working weekends. About the
Role:
You will engage patients through motivational interviewing, use data to support care for high-risk populations, prepare clinical data for review, prepare workflows to support team-based care, and evaluate progress toward goals.
Key responsibilities include:

Education and Skills:
Bachelor's degree or equivalent professional experience 1-5 years' experience preferred Experience in community health, patient resource coordination or equivalent Computer skills, clerical skills, data entry and organization Excellent written and verbal communication, and problem solving skills Able to read, write and speak the English language. A second language of Spanish is preferred

What We Offer:
Compensation:
$28.00 an hour based on a full time, 40 hour work week.
