Job Description
Salary Pay Range:
$23.00-$25.75 The treatment coordinator is responsible for the coordination of rehabilitative clinical services to patients receiving services from the People Inc Article 16 Clinic and its certified satellite Buffalo and Rochester locations. Prepares for and participates inOPWDD A16
surveys as indicated. Maintains appropriate records, statistical reports, etc. to ensure appropriate rehabilitative treatment activity and compliance. Ensures that confidentiality is strictly maintained concerning all agency-related information.ESSENTIAL FUNCTIONS/RESPONSIBILITIES
Ensure the admission/intake process procedures are followed and the required paperwork is completed. Ensure service requested is not currently being provided elsewhere and the referral form is reviewed and approved by the medical director. Collect necessary paperwork for a patient to participate in Article 16 rehabilitative services. Interview patient and collateral (as indicated) during an intake appointment. This includes review of the People Inc Article 16 procedures and required documentation that the treatment coordinator has collected. Document the intake visit electronically and sends to Medical Director for review and approval through signature. Provide the clinicians with a schedule of plans due. Assist in maintaining the treatment plans. Distribute documentation as indicated to CCOs and referral sources according to compliance guidelines. Follow up with any lapses in completing plans in a timely manner with appropriate personnel, including billing and compliance, if necessary, to ensure appropriate resolution. Communicate with a person's support members and providers to ensure documentation accurately reflects services being provided. Ensure that the patient is receiving the service(s) as scheduled/recommended. Follow-up on referrals in a timely manner. Evaluate the patient's/collaterals' satisfaction with the service(s) via satisfaction surveys and follow up with any issues and concerns in a timely matter. Document follow up that was required to maintain satisfaction of the service delivery process. Maintain electronic clinical record as it pertains to Article 16 services including documentation of communication with CCO's and other approved contacts. Ensure that through the Annual Physician Reassessment and Clinical Treatment Review that continued need for service is provided. Participate in peer-to-peer audits Coordinate all the rehab services provided to the patient through the Article 16 clinic. Participate in Quality Assurance Activities.Team Functions and Responsibilities:

Baccalaureate degree or a license as a registered nurse 3-5 years of experience working with the Developmentally Disabled or relatable administrative workSUPERVISORY RESPONSIBILITIES
While this position has no direct supervisory responsibility, the treatment coordinator will provide guidance to a multidisciplinary team responsible to provide services to the Developmentally Disabled.
