Position Summary:
The Caregiver Support Western Queens Program provides information, referrals, counseling and respite to family caregivers - those family members or friends who are providing care to physically disabled or cognitively impaired older adults, grandparents/kinship caregivers raising children, and older adults caring for a disabled adult. Respite service provides temporary relief from caregiving responsibilities on a limited basis and can include in-home care or adult day care services. The Care Manager supports caregivers and care recipients by assessing needs, developing individualized care plans, and ensuring access to appropriate services and supports. This role provides ongoing guidance, counseling, and education to strengthen caregiver capacity, promote long-term planning, and enhance overall client well-being, and fostering engagement through support groups and training opportunities. Essential Duties and Responsibilities The essential functions include, but are not limited to the following: Intake, Assessment & Care Planning
- Conduct intake and assessment screening of both caregiver and care receiver, evaluating mental and physical health needs, support systems, and financial eligibility for benefits, entitlements, and respite services.
- Perform home visits to caregiver clients and care receivers to evaluate unmet needs.
- Collaborate with the caregiver to develop a plan of care and implement constructive short and long-term interventions through counseling, education about caregiving supports, long-term planning, and assistance with benefits.
- Conduct periodic reassessments of caregiver and care recipient needs to update care plans as circumstances change. Linkages, Referrals & Benefits
- Link caregivers to other service providers such as mental health, medical care, and financial management services as needed.
- Assist caregivers and care recipients with applications for benefits and entitlements (e.g., Medicaid, SNAP, SCRIE, home care services).
- Monitor and follow up on referrals to ensure clients successfully connect with and access recommended services.
- Coordinate client respite and supplemental services.
- Respond to crisis situations involving clients, including coordinating emergency services or referrals as needed. Education, Groups & Training
- Lead one or more support groups, providing emotional support and psychoeducation about caregiving and self-care.
- Support and facilitate client training events on topics that enhance caregiving capacity, such as self-care, health and wellness, legal planning, and assistance with daily care needs.
- Develop and disseminate informational materials (flyers, newsletters) about caregiver resources and upcoming events.
- Assist with planning, marketing, and logistics of caregiver support groups, workshops, and training events. Outreach & Community Engagement
- Conduct outreach to community organizations, senior centers, healthcare providers, and faith-based organizations to identify and recruit eligible caregivers for the program.
- Provide translation/interpretation support for Non-English-speaking clients during appointments, home visits, or when working with other service providers, as needed. Documentation, Reporting & Compliance
- Provide timely documentation and maintenance of client files and progress notes.
- Track and report program outcomes and statistics for grant compliance and funder reporting requirements (e.

- Maintain up-to-date knowledge of community resources, entitlement programs, and eligibility criteria. Education and Experience Requirements
- MSW, or equivalent preferred, or minimum BSW/BA with at least 1 year of relevant experience in social services, assistance with benefits a plus.
- Experience working with older adult populations and families preferred.
