This is a senior leadership role with full operational and administrative accountability for a MassHealth-contracted provider of Adult Foster Care (AFC) and related community-based services. The General Manager will be the primary point of accountability for all aspects of the organization's performance — including clinical quality, regulatory compliance, workforce management, financial health, and the experience of the members and caregivers we serve. Reporting to the CEO of Vizx Corp, this leader will guide the organization through a period of transition while maintaining the quality of care and the community relationships that define its reputation. We are looking for a leader who understands the operational demands of a MassHealth AFC environment — from Multidisciplinary Team (MDT) structure and CARF accreditation obligations to caregiver support and prior authorization workflows — and who brings the people skills to sustain trust and stability across staff, members, and partners throughout organizational change. Key Responsibilities Operations & Service Delivery Oversee all daily operations across the organization's AFC and home-based service programs Ensure consistent, high-quality service delivery in accordance with MassHealth AFC regulations (130 CMR 408.000) and CARF accreditation standards Maintain the Multidisciplinary Team (MDT) structure required under MassHealth—including Program Director, RN/LPN, and Care Manager functions—and ensure each role is staffed, supported, and performing effectively Oversee caregiver recruitment, evaluation, training, and ongoing support to maintain program quality and member safety Ensure timely completion of clinical assessments, prior authorizations, and care plan documentation Compliance & Regulatory Oversight Maintain full compliance with MassHealth AFC and Group AFC (GAFC) regulations, billing rules, and administrative standards Ensure HIPAA compliance and proper data-handling practices across all program areas Oversee CARF accreditation readiness, including policies, records, internal monitoring tools, and mock survey preparation Manage incident reporting, corrective action planning, and quality management obligations under MassHealth requirements Monitor regulatory changes from EOHHS and MassHealth and implement policy adjustments proactively Workforce Leadership & Culture Lead, develop, and retain a high-performing team across clinical, administrative, and care management functions Establish clear expectations, provide ongoing feedback, and create a work environment grounded in accountability and mutual respect Conduct employee listening sessions and respond visibly to staff concerns, particularly during periods of organizational change Identify and mitigate workforce risks, including key talent retention and succession planning Foster a culture where caregivers and staff feel valued, informed, and supported Financial Management Manage site-level budget, cost structure, and vendor relationships Oversee revenue cycle performance, including MassHealth billing, claims, accounts receivable aging, and prior authorization compliance Identify opportunities for financial efficiency and service-line growth Provide regular financial performance reporting to Vizx Corp leadership Stakeholder & Community Relations Maintain and strengthen relationships with MassHealth, referral partners, PCPs, and community-based organizations Serve as the primary organizational representative for external partners and regulatory contacts Communicate proactively with members, caregivers, and families regarding any service or program changes Organizational Transition Serve as a visible, accessible, and stabilizing leader for staff and caregivers during a period of organizational change Partner with Vizx Corp's Director of Administration & Operational Systems on operational alignment, system onboarding, and policy harmonization Support the development and execution of a unified operational framework aligned with Vizx Corp standards, while preserving program strengths built by this organization Qualifications Required 5+ years of progressive leadership in home health, senior care, AFC, or community-based healthcare services Demonstrated knowledge of MassHealth billing, compliance, and AFC program operations Experience overseeing a Multidisciplinary Team (MDT) in a MassHealth AFC or GAFC setting Track record of leading organizations through regulatory audits, CARF accreditation, or similar quality reviews Strong workforce management skills with the ability to build trust, address performance issues, and reduce turnover Financial literacy in healthcare revenue cycle management, including payer billing and AR oversight Exceptional communication skills; comfortable engaging with staff, members, caregivers, regulators, and leadership Preferred Direct experience as a Program Director or GM of a MassHealth
