This is an ON-SITE position located in Queens, NY Applicants MUST have healthcare or medical office experience This position is heavily phone-based and similar to a healthcare call center environment Bilingual or multilingual candidates are highly preferred (English, Russian, Spanish, or other languages) About Us We are a growing healthcare support company that partners with physicians and medical facilities to provide Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services. Our mission is to improve patient health outcomes through consistent communication, patient education, care coordination, and compassionate support. We maintain strict HIPAA compliance and ensure all patient information is handled with the highest level of confidentiality and security. Position Summary We are seeking a compassionate, organized, and reliable Care Coordinator / Healthcare Call Center Representative to support patients enrolled in CCM and RPM programs. This is a high-volume outbound calling position focused on conducting monthly patient follow-up calls, documenting patient concerns, coordinating care, and assisting patients with healthcare-related needs. The ideal candidate is comfortable speaking on the phone for most of the day, has strong communication skills, and can professionally handle patient interactions in a fast-paced healthcare environment. Responsibilities Conduct monthly outbound follow-up calls with patients and/or family members Discuss patient health status, medications, symptoms, and care needs Educate patients regarding medical conditions and treatment plans Develop and update individualized care plans Coordinate appointments and communicate with physicians and specialists Accurately document all patient interactions and updates in the system Monitor patient progress and provide ongoing support Maintain HIPAA compliance and patient confidentiality at all times Deliver professional, compassionate, and respectful customer service Meet daily and monthly call/documentation expectations Requirements
REQUIRED
Medical or healthcare background experience (medical office, clinical, healthcare support, care coordination, medical assistant, nursing, call center healthcare experience, etc.) Comfortable handling high-volume phone calls daily Strong verbal and written communication skills Professional phone etiquette and customer service skills Proficient in Microsoft Office and basic computer applications Strong multitasking and organizational abilities Ability to work in a fast-paced office environment Reliable, compassionate, patient-focused, and detail-orientedPreferred Qualifications Experience with CCM or RPM programs Experience in a healthcare call center environment Knowledge of HIPAA compliance and healthcare documentation Bilingual or multilingual skills Experience using EHR/EMR systems Strong documentation and follow-up skills Why Join Us?
