Job Overview:
The Care Management Supervisor oversees and directs the Care Managers under his/her assignment and works in conjunction with the Director of Care Management to ensure that all practices and programs are aligned with the company vision.
Essential Responsibilities:
Provides day-to-day oversight of Care Management operations Directs the policies and procedures and standard for Care Management operations Responsible for hiring of the staff to ensure that they are appropriately oriented, trained and supervised. Assigns caseload to CMs and oversees caseload to ensure timeliness of deadlines/expectations Analyze data to measure individual and team performance and department processes. Provides leadership for care management policy initiatives, programs, and other innovations Communicate with members regarding dissatisfaction or issues related to CM Works with the Director of Care Management on a variety of critical tasks, including the management of one or more focused clinical initiatives. Leads individual and team meetings Participates in management meetings Other responsibilities as deemed necessary by the
Director of Care Management and COO Required Education, Experience, and Licenses:
The appropriate background in health care and/or long-term care Valid New York State, unrestricted, RN license. Bachelors Degree (BSN) preferred. Excellent communication, written and analytical skills. Computer savvy.
