Full Job Description Total Compensation:
$165k-$180k Are you an experienced nursing leader who is passionate about improving patient outcomes and helping children with medically complex conditions receive exceptional care? We are seeking a compassionate and strategic Director of Care Management to lead an experienced inpatient case management team at a nationally recognized pediatric specialty healthcare organization. In this leadership role, you'll help shape the patient journey from referral through discharge while collaborating with physicians, nursing leaders, rehabilitation specialists, social work, and patient access teams. What You'll Do Provide day-to-day leadership and oversight of the inpatient RN Care Management department. Develop strategies that improve patient flow, care coordination, and timely admissions and discharges. Partner with physicians and interdisciplinary leaders to enhance transitions of care across the continuum. Review incoming referrals to determine clinical appropriateness while evaluating medical, rehabilitation, and psychosocial needs. Build collaborative relationships with referring hospitals and healthcare providers to streamline referral and admission processes. Analyze utilization data, referral trends, length of stay, and discharge barriers to identify opportunities for operational and clinical improvement. Create and implement policies, workflows, and best practices that support efficient care coordination and regulatory compliance. Work closely with Patient Access, Social Work, Nursing Education, and other departments to improve the patient and family experience. Support continuous quality improvement initiatives through data analysis, performance monitoring, and evidence-based practice. Assist with developing innovative approaches that improve efficiency while supporting quality outcomes and financial performance. Mentor, coach, and develop members of the care management team while fostering a collaborative, patient-centered culture. Perform additional leadership responsibilities as needed. Qualifications Current New York State Nurse Practitioner license required. Master's degree in Nursing (MSN) required. Previous pediatric clinical experience required. Prior Pediatric Intensive Care Unit (PICU) experience required. Leadership or management experience within case management, care coordination, or utilization management strongly preferred. Background in quality improvement, process improvement, or project management is a plus. Why Join Us?
