The Hospice Clinical Supervisor leads the hospice clinical team to utilize a collaborative process to assess, plan, implement, monitor, and evaluate the options and services required to meet hospice patient/family physical, emotional, psychosocial, and spiritual needs while coordinating available resources to promote quality and cost-effective outcomes through ongoing audits of medical records and clinical documentation to continually improve data quality in accordance with company standards and federal, state, and local standards, guidelines, and regulations Essential Activities and Tasks Operations Management and Administration
- 55% Facilitates clinical team with the coordination and oversight of all direct and indirect patient/family care services ensuring accurate evaluation of patient/family physical, emotional, psychosocial and spiritual needs, and development and implementation of comprehensive care plans that address those needs by following current hospice nursing professional standards of practice and agency approved protocols.
- 30% Audits and maintains current and accurate medical records and clinical documentation in accordance with company standards and federal, state, and local standards, guidelines, and regulations.
- 15% Supervises the hospice clinical team to operate within the allocated resources and budgeted goals, efficiencies and productivity standards.
- Up to 8 hours/day Standing
- Up to 8 hours/day Walking
- Up to 8 hours/day Lifting, transferring, pushing or pulling patients or equipment/supplies
- Up to 50 pounds Driving
- Up to 6 hours/day Travel % / Overnight Travel
- Occasional Work weekends, evenings, and holidays
- As needed for coverage On-call availability
- Occasional Subject to patients with various disease processes
- Occasional May be exposed to infectious waste, disease, conditions, etc. including exposure to the AIDS and Hepatitis B viruses
