Job Description

Shift:
Part-time Hours TBD Care Manager will coordinate, refer, advocate as outlined in the job description. Will complete Needs Assessments within guidelines and begin discharge planning at first session with a client. Complete ROI and send Letters of Presence, facilitate housing appointments, facilitate video court sessions, collaborate with treatment teams and attend treatment team meetings, collaborate outside community partners. Additional duties as assigned. Must hold a minimum of a CM1 certificate
