About the Role The Admissions Coordinator, Outpatient is the first point of contact for individuals seeking treatment, playing a critical role in connecting patients to care. This position manages the intake process from initial inquiry through assessment scheduling and documentation, ensuring a compassionate, efficient, and organized experience that supports timely access to services. What You'll Do (Please note this is not an exhaustive list of responsibilities and duties) Respond to inquiry calls, voicemails, and provide accurate information about services and admissions Schedule and coordinate patient assessments, including warm handoffs when appropriate Create and maintain patient records in Salesforce and the electronic health record (EHR) Complete and track intake documentation, including admission forms and required paperwork Monitor call-back lists and document all interactions in the EHR Coordinate with outpatient teams to ensure smooth patient transitions and follow-up Use call center software and Microsoft Teams to support communication and workflow tracking Direct patients to appropriate levels of care or community resources based on initial screening Follow established protocols to ensure compliance and consistency in intake processes
What We're Looking For Qualifications:
o High School diploma or equivalent, required o 3+ years of customer service experience, preferred o Data entry experience, preferred
Skills & Abilities:
o Strong typing and data entry accuracy o Excellent communication and customer service skills o Ability to manage multiple tasks in a fast-paced environment o Professionalism and strict adherence to confidentiality standards o Comfort working with EHR systems, call tracking software, and Microsoft Teams o Strong organization and attention to detail Ability to pass a
