Job Description

Job Overview We are seeking a dynamic and detail-oriented Patient Care Coordinator-Referral Specialist to join our healthcare team. In this vital role, you will serve as the last point of contact when patients check out for their appoinments. This Position manages and coordinates outside specialty appointments and ensures continued continuity of patient care in a timely and professionally manner; maintains positive relationships for collaboration with area hospitals, ancillary departments, and specialty physicians, nursing facilities, patient families and the general public. This role also assists with patient communication by phone and appointment scheduling as needed. Responsibilities Check out patients
  • by assisting to schedule a follow up appointment, collecting Co-Insurance payments or providing other assistance as needed. Checking for any orders or visit summaries patients may need before leaving. Process Referrals
  • by coordinating the scheduling of appointments for patients with the specialist's office and sending medical records when necessary. Notify the patient of the appointment or which office will be contacting them for an appointment. Track Referral Status
  • Maintain a list of pending referrals and tracking them to completion by confirming receipt of consult notes Messages
  • take messages using the electronic health record and include all pertinent information or transfer the caller to the appropriate person or department. Schedule appointments
  • As patients call in for a variety of visit types using practice management software.
Also responsible for scheduling appointment follow-ups as needed per telephone encounters Experience Prior experience working in a medical front office, Referrals Dept, or Medical Records Dept strongly preferred. Strong understanding of HIPAA regulations to protect patient confidentiality at all times. Electronic Health Record Experience preferred This is a full-time position with a set schedule of Tuesday-Friday 7:00am to 5:30pm. Saturdays, Sundays, and Mondays OFF.
