Portland Mental Health & Wellness is a thriving private practice and training practice with four locations in Portland, as well as one location in Salem. We are a collaboration of psychologists, psychiatric prescribers, counselors, marriage and family therapists and our trainees. We strongly emphasize trauma informed care, cultural competency, establishing comfort and safety for patients, and coordination of care with primary care doctors, specialists, pediatricians, schools, and other mental health clinicians. — Patient Care Coordinator (PCC)

Primary Function:
The Patient Care Coordinator (PCC) is an administrative support role responsible for coordinating patient scheduling, internal and external referrals, and broader administrative tasks across PMHW. This role supports efficient patient access to care by utilizing complex scheduling workflows, facilitating continuity of care, and collaborating closely with clinical, billing, and administrative teams. The PCC team also contributes to operational consistency, staff support, and process improvement initiatives across the organization.
Essential Functions:
Greet and assist patients professionally across phone, email, and in-person interactions Schedule new, returning, and internal transfer patients for ongoing therapy and related services Coordinate complex scheduling needs across providers, specialties, and locations Manage and track internal and external referrals, including clinical fit, follow-up, and timeline management Maintain accuracy and confidentiality in patient charts, scheduling systems, and related documentation Collaborate with clinical, billing, credentialing, and administrative teams to ensure continuity and accuracy of patient care coordination Maintain working knowledge of how insurance credentialing, billing requirements, and provider status impact scheduling and patient access to care Facilitate transfer-of-care planning when providers take leave or separate from the practice Provide cross-coverage and administrative support across departments and office locations as needed Identify workflow inefficiencies and collaborate with leadership and team members on process improvement efforts Support onboarding and training of new front desk and patient care coordination staff Respond to patient concerns appropriately and escalate issues when necessary Assist with medical records coordination, releases of information, and other administrative support tasks as needed Maintain organization and operational readiness of front desk and administrative areas
Minimum Qualifications:
Preferred:
at least 1 year of experience in healthcare administration or similar role Demonstrate competence and humility in terms of race, ethnicity, gender, sexual orientation, religion, class, ability et al Demonstrate commitment to the preservation of a workplace which embraces diversity Demonstrate commitment to confidentiality of patient and employee information Communicate professionally and effectively with patients and clinicians, always providing stellar customer service Answer phone systems with excellent etiquette

Work Environment:
When working from a PMHW office, the position is in a well-lit office environment. Varied activities including sitting, standing, walking, reaching, bending, and lifting.
Mental and Physical Requirements:
Involves sitting approximately 90 percent of the day, walking or standing the remainder. Ability to tolerate mental and emotional distress as an anticipated aspect of the position. May require working under stressful conditions.
