The Medical Coordinator is responsible for overseeing and coordinating all healthcare-related services for individuals residing in the group home. This position ensures accurate medication management, coordinates healthcare appointments, maintains compliance with medical documentation requirements, and provides medication-related training to staff. The Medical Coordinator works closely with healthcare providers, pharmacies, and direct support staff to ensure individuals receive safe, timely, and high-quality care. Key Responsibilities Review, update, and maintain all medication records in Therap (Medication Administration Records - EMAR). Coordinate and track all medical, dental, and specialist appointments, including scheduling and follow-ups Maintain accurate and up-to-date medical records, medication logs, and health documentation in THERAP Serve as a liaison between physicians, pharmacies, nursing staff, management, and families/guardians to ensure continuity of care Monitor individuals' health needs, follow prescribed treatment plans, and ensure provider recommendations are implemented Oversee medication administration practices, including timely delivery, documentation, and tracking of refusals, omissions, errors, and refills Document and report medical incidents, including missed doses, medication errors, and health concerns, to nursing consultants and supervisors Ensure compliance with all ODP, DHS, and applicable state regulatory requirements Provide guidance, training, and support to Direct Support Professionals on medical protocols and best practices Monitor Medication Inventory and ensure timely refills. Qualifications Experience working with individuals with intellectual or developmental disabilities (IDD), or direct support professional role. Experience in medical coordination, healthcare, nursing, pharmacy, or a related field preferred. Experience using Therap or similar electronic health record systems is preferred or willingness to learn. Knowledge of medication administration procedures and healthcare documentation especially in a group home setting. Familiarity with HRST or willingness to learn. Knowledge of ODP regulations or willingness to learn. Strong organizational and time-management skills. Excellent communication and interpersonal skills. Ability to manage multiple priorities while maintaining accuracy and confidentiality. Working Conditions Full-time salaried position. Primarily office-based with travel for appointments or coordination activities. May be required to assist the Programs Department as needed to support operations and ensure continuity of services This position plays a critical role in ensuring individuals receive coordinated, safe, and compliant healthcare services while supporting staff through training and ongoing medical oversight. It involves regular interaction with individuals receiving services, medical professionals, and internal staff.
