The Clinical Supervisor for the Children and Adult High Risk Care Management Team is responsible for assisting the care management staff in identifying members at risk for recidivism, discontinuous care, or as members of priority or special needs populations who present with complex needs for coordination of their behavioral health services with other aspects of their care. The Clinical Supervisor is responsible for the supervision of staff that assist assigned members to care at all levels of the continuum, and for providing any and all care coordination as needed. The Clinical Supervisor executes these responsibilities consistent with applicable Policies and Procedure of Erie County's Managed Care provider, Community Care Behavioral Health's (CCBH). The Clinical Supervisor is able to engage in independent problem solving that results in creative and effective solutions to member problems. They have demonstrated knowledge of the service delivery system, community resources, clinical treatment, and case management.
Duties and Responsibilities:
Assigns direct work for care managers, specifically selecting staff to match his/her area(s) of expertise for each individual member in a person centered care management model. Trains, educates, supports and supervises the Children and Adult High Risk Care Management Team. Works as part of the supervisory team to provide clinical expertise and knowledge to the other members of the team. Assures supervisory access for all care management staff as needed for case collaboration and care planning. Collaborates with providers and others in order to obtain initial assessment, treatment planning and aftercare planning for members. Conducts clinical reviews, medically necessary service authorization, and care coordination for all assigned members receiving behavioral health services. Supervises collection of information regarding the delivery and outcomes of services to members. Recommends modifications to plan policies and procedures for the improvement of the delivery of services to members. Develops specific outreach plans for assigned members who do not maintain regular contact with their behavioral health provider as recommended contributing to frequent crises, recidivism, and interfering with maximum benefit from available care. Maintains an understanding of behavioral health benefits and remains current on covered benefits, limitations, exclusions, and policies and procedures, in regards to services. Provides members, providers, and other stakeholders with accurate information concerning behavioral health care benefits and coverage under HealthChoices. Facilitates linkages and encourages coordination of care for members and families between primary care and behavioral health providers, and other social service or provider agencies as needed, to develop and coordinate service plans. Receives and responds to complex and crisis calls. Works with Member Services, Network Management and Quality Management staff to assure that systematic revisions to improve services are developed and implemented. Coordinates, reviews and maintains daily logs for reporting purposes and for weekly preparation and analysis of trending reports. Identifies provider issues and recommendations for improvement.
Qualifications:
Master's Degree in a mental health-related field required, as well as Licensure as RN, LSW, Licensed Psychologist, LPC Minimum of 5 years experience in behavioral health or substance abuse treatment. Community based experience preferred. Established knowledge of and work experience with the Medicaid affiliated population desired.
