We're seeking a compassionate and organized Medical Coordinator to join our team at Heart to Heart. In this role, you'll provide essential healthcare coordination and support services for individuals with developmental disabilities while fostering a nurturing and inclusive environment.

Key Responsibilities:

Healthcare Coordination & Management Coordinate and schedule healthcare services, medical appointments, examinations, and procedures for individuals with developmental disabilitiesAttend medical appointments with individuals to obtain necessary documentation and informationMaintain detailed, accurate medical records and appointment documentationPrepare and manage monthly medical appointment calendarsCoordinate with emergency services and healthcare providers as neededProvide assistance with ADLs including hygiene, diapering, meals and other tasks as needed Medication Management Oversee medication administration protocols and staff trainingPrepare and submit monthly medication ordersConduct regular medication audits to ensure proper supply and complianceMonitor medication administration documentation using Therap systemProvide medication education to individuals regarding their prescriptions Program Support & Documentation Assist in developing and updating individualized health plansPrepare and submit reports in compliance with ODP regulationsImplement emergency response protocols for medical situationsDevelop and implement house menus considering dietary needs and preferencesConduct safety orientations and emergency drills for residents Community Integration & Care Create strategies to maximize resident inclusion in community lifeTransport individuals to community destinations as neededEstablish a warm, nurturing environment for residentsMaintain interactive relationships that reflect Heart to Heart's values and principles
