Job Description

strong communication skills, both Countrywide Home Care is looking for Case Coordinator /Scheduler with Coordination experience in home care. An excellent candidate is organized, detail-oriented, self-motivated, professional, and has strong communication skills, both on the phone and in person. The candidate must be comfortable dealing with people, performing administrative tasks, and multitasking with speed and accuracy. Hours for this position are Monday through Friday from 9 a.m. to 5 p.m., with additional on-call responsibility. Responsibilities and Duties Providing coordinated care to patients by developing, monitoring, and evaluating their home health care plans. Ensure that the home health aides give a high level of care to the patient. Scheduling agency home health care aides accordingly and ensuring agency aides work their scheduled shift. Managing last-minute call-outs and finding appropriate coverage. Listening to clients' needs. Manage a Caseload of up to 70 patients. Review the care plan with patients and caregivers. Resolve caregiver and client grievances and complaints Ensure caseload retention Participate On-call Qualifications Experience with HHA Exchange and EVV Strong typing and computer skills Demonstrated capability in maintaining strict confidentiality. High Communication Skills. Strong computer skills, including Word and Excel;
