Job Description

The Patient Care Coordinator functions as an integral role in facilitating provision of quality patient care by coordination of all patient services. The following serves as a representative summary of the major duties and responsibilities of the Patient Care Coordinator position, however individuals may be required to perform additional duties as requested not included in this general description.
ESSENTIAL JOB FUNCTIONS
1. Answering all incoming phone calls (including directing clinical or patient phone calls as necessary to on-call nursing staff for proper follow-up) 2. Intake and data entry of all patient referrals, including information received via phone, email, fax, or third party vendor (All Scripts, ROI, etc.) 3. Scheduling Starts of Care, evaluations, and follow-up visits in accordance with agency policy and required regulations 4. Assisting with coordination of daily schedules by working closely with full-time and per diem interdisciplinary field staff 5. Establishes and maintains positive working relationships with current and potential referral sources 6. Maintains effective communication between clinical/office staff, patients/caregivers, and existing/potential referral sources 7. Additional tasks as requested by Intake and Scheduling team members during down time Position Qualifications 1. At least one year experience in health care office setting, preferably in home care operations 2. Ability to market aggressively and deal tactfully with customers and the community 3. Knowledge of governmental regulations, CHAP standards, and private payer practices 4. Demonstrates good communication, negotiation, and public relations skills 5. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
