Behavioral Health Care Manager Location:
Hope Shelter 220 N.
Broad Street Position Overview :
Serve as a 'bridge' between homeless shelter residents and behavioral health, medical, and social service resources.

Focuses on engagement, assessment, and coordination of care to connect individuals with varying degrees of behavioral health needs with mental health, substance use, housing, and community-based resources. Works collaboratively to assess needs and improve overall client outcomes. Conduct screenings and needs assessments related to mental health, substance use, and social determinants of health. Facilitate warm handoffs to appropriate levels of care as needed. Deescalate emotionally heightened individuals; connect with crisis services as needed Engage with shelter residents to build trust and rapport Provide referrals and behavioral health services, primary care, housing programs, and other community resources Assist in navigating complex systems, including benefits, insurance, and entitlement programs Coordinate care with internal clinicians, case managers, and external service providers We are seeking a dedicated professional who : Can work independently and adapt to a dynamic environment. Will engage with a vulnerable population and link them with needed resources. Can cultivate positive relationships with clinical teams. Is mission-driven, demonstrating empathy and respect for the individuals we serve.
Training & Experience :
Bachelor's degree in social work, Human Services, Counseling, Psychology, or a related field required. Two years' previous experience providing outreach or case management services in a community setting.
Schedule :
Full-Time 37.5/week 10AM-6
PM Location:
North Philadelphia, accessible to SEPTA
