Role Overview We are seeking a proactive and experienced Home Care Coordinator to help expand our community impact and support our growing client base. In this role, you will connect clients and caregivers to services, manage the intake process, develop individualized care plans, coordinate caregiver schedules, and cultivate strong referral relationships within the healthcare community. This is a hands-on position that combines client support, care coordination, caregiver recruitment, community outreach, marketing, and business development. Candidates must have experience in home care. Key Responsibilities Community Outreach, Marketing & Referral Development Conduct local community outreach and marketing activities to promote agency services. Visit hospitals, rehabilitation centers, assisted living facilities, physician offices, senior living communities, and community organizations. Introduce agency services to healthcare professionals, referral sources, and community partners. Build and maintain professional relationships with case managers, social workers, discharge planners, physicians, and other healthcare providers. Follow up on referrals, inquiries, and prospective client leads in a professional and timely manner.. Recruit caregivers through outreach efforts, partnerships, networking, and advertising campaigns. Help distribute marketing materials and agency information throughout the community. Track outreach activities, referrals, enrollments, and caregiver recruitment efforts. Provide updates on outreach activities, referral opportunities, and community engagement efforts. Represent the agency professionally at community events, healthcare meetings, and networking functions. Assessment, Care Planning & Client Intake Conduct initial assessments of clients' physical, medical, emotional, and personal care needs. Develop individualized care plans tailored to each client's needs, preferences, and health requirements. Guide clients and families through the intake process, including documentation, eligibility verification, and program enrollment. Monitor client needs and update care plans as necessary to ensure quality service delivery. Scheduling & Care Coordination Assist in scheduleing caregivers for client shifts and visits and adjust schedules as needs change. Ensure continuity of care by assigning consistent caregivers whenever possible. Coordinate with internal teams to support caregiver onboarding, placement, and retention. Address scheduling conflicts and staffing needs promptly and effectively. Communication & Liaison Serve as the primary point of contact between clients, families, caregivers, and referral sources. Communicate care instructions from healthcare providers and family members to caregivers. Provide updates and reports regarding client status and care services. Respond to client, family, and caregiver concerns in a timely and professional manner. Administrative Duties Maintain accurate care records and documentation, including schedules, service notes, incidents, and care-related activities. Manage billing, insurance documentation, authorizations, and related administrative tasks as needed. Ensure compliance with agency policies, HIPAA requirements, and state regulations. Support quality assurance and continuous improvement initiatives. Qualifications Minimum of 1 years of healthcare experience required. Previous experience in home care, home health, care coordination, or case management preferred. Experience in community outreach, healthcare marketing, business development, or referral relationship management is highly preferred. Strong communication, organizational, and relationship-building skills. Ability to manage multiple priorities and work independently in a fast-paced environment. Comfortable handling sensitive and confidential client information. Reliable transportation and a valid driver's license are required. Bilingual or multilingual skills are highly preferred to effectively communicate with diverse clients, families, and community partners. $18.00 - $22.00 per hour
