Job Description

Job Overview Our mental health clinic is seeking a dedicated, compassionate, and organized Patient Care Coordinator to join our team. This role is essential in providing excellent support for those we serve and acts as a liaison between patients, families, physicians, and multidisciplinary team members. You will work directly with patients/clients and also assist with administrative responsibilities - facilitating communication, managing logistics, and supporting optimal clinical outcomes. The ideal candidate will possess strong communication skills, be proficient in administrative skills, and have experience in customer service. As the first point of contact for those we serve, you will play a crucial role in creating a welcoming environment. Care Coordination & Navigation Serve as the primary point of contact for patients and families throughout the care process. Coordinate appointments, diagnostic tests, procedures, and follow-up visits, ensuring timely access to care. Facilitate referrals and transitions between primary care, specialty care, and community resources. Monitor and support adherence to treatment regimens, including medication schedules and follow-up care. Admin Responsibilities Greet and assist clients and visitors in a professional manner. Manage incoming calls and inquiries using phone systems efficiently. Schedule appointments and manage calendars for staff members. Perform clerical duties such as data entry, and maintaining office records. Provide customer support by addressing client questions and concerns promptly. Assist with administrative tasks as needed to support the overall operations of the clinic. Experience Previous experience as a receptionist or in an administrative role is preferred. Previous experience in a social employment setting is also preferred. Excellent time management skills to prioritize tasks effectively. Familiarity with medical receptionist duties is a plus. Ability to operate computerized systems for scheduling and record-keeping. Strong communication skills, both verbal and written, to facilitate effective interactions with clients and team members. • This position will be part time, initially, with the potential to transition to full time. Pay:
