Job Description
Behavioral Health Transfer Coordinator (RN/LPN) - 100% Remote We're hiring licensed RNs and LPNs with behavioral health experience to join a fully remote Transfer Center team. This role is responsible for reviewing referrals, conducting clinical assessments, and coordinating patient admissions in a fast-paced, high-volume environment.
Key Responsibilities:
Manage and triage incoming behavioral health referrals across multiple platforms and systems Perform clinical intake assessments in collaboration with referring nurses and providers Determine patient appropriateness for services based on established admission criteria Coordinate the admissions process and ensure patients are placed in the most suitable level of care Serve as a liaison between hospitals, physicians, and internal clinical teams to collect and verify critical patient information Assess and prioritize referrals based on acuity, urgency, and clinical needs Maintain accurate, timely documentation in internal systems to support compliance and continuity of care Partner closely with team members to promote efficient workflows and support timely patient placement

Requirements:
Active RN or LPN license + compact license 2+ years of clinical experience Behavioral health experience required (admissions/intake preferred) Strong decision-making, communication, and technical skills Ideal candidates are analytical, adaptable, and thrive in a fast-paced environment where quick, informed decisions are key.
