Job Description

Mental Health organization is excited about hiring an experienced Care Manager for community-based work. Must possess strong written and verbal communication skills. Have a history of working with underserved communities and be comfortable with mental health and/or physical health populations. ```Duties```
  • Provide case management services to individuals and families in need
  • Conduct assessments to determine client needs and develop individualized service plans
  • Coordinate and monitor the delivery of services to clients
  • Assist clients in accessing community resources and support services
  • Advocate for clients' rights and ensure their needs are met
  • Collaborate with other professionals, such as social workers and healthcare providers, to provide comprehensive care
  • Maintain accurate and up-to-date case records ```Requirements```
  • Bachelor's degree in social work, psychology, or a related field
  • Experience in case management or a related field
