We are a dedicated community services organization committed to breaking the cycle of homelessness. Operating across three counties, we provide a compassionate bridge from life on the streets to the stability of a permanent home. Our mission is to meet individuals exactly where they are, offering a path forward through intensive support, resource navigation, and unwavering advocacy. We are looking to fill a cook position. Specific job duties and responsibilities are subject to change depending on client needs.
Location:
Northern California Reports to: VP of Operations Role Overview The Clinical Director is a pivotal leadership role designed to oversee the clinical integrity, quality of care, and operational compliance of Enhanced Care Management (ECM) and Community Supports (CS) under the CAlAIM initiative. You will lead a multidisciplinary team of Lead Care Managers and Clinical Supervisors to ensure that Medi-Cal members with complex needs- including those experiencing homelessness, SMI, or SUD- receive high-touch, person-centered coordination that stabilizes their health and housing. Key Responsibilities 1.
Clinical Leadership & Strategy Program Oversight:
Drive the clinical strategy for ECM and CS services, ensuring interventions are trauma-informed and culturally competent.
Complex Case Consultation:
Provide clinical guidance on "high-utilizer" cases, helping teams navigate barriers in the healthcare and social service ecosystems.
Health Equity:
Implement strategies to reduce health disparities within the assigned region. 2.
CalAim Compliance & Quality Assurance Regulatory Alignment:
Ensure all documentation meets Department of Health Care Services (DHCS) and Managed Care Plan (MCP) standards.
Audit Readiness:
Lead the preparation for clinical audits and "Model of Care" reviews conducted by contracted health plans.
Outcome Tracking:
Monitor Key Performance Indicators (KPIs) such as engagement rates, housing placements, and reductions in ED visits. 3.
Operational Management Staff Supervision:
Recruit, train, and supervise Supervisors and Lead Care Managers.
MCP Liaison:
Act as the primary clinical point of contact for Managed Care Plans.
Network Integration:
Build and maintain relationships with local hospitals, behavioral health departments, and housing authorities.
Required Qualifications Licensure:
Active, unrestricted clinical license in California ( LCSW, LMFT, LPCC, or RN ).
Experience:
Clinical Leadership of healthcare administration. Direct experience with Medi-Cal populations and managed care environments. Proven track record in Case Management , Housing First modules , or Behavioral Health.
Knowledge:
Deep understanding of the CalAim roadmap, specifically ECM core services and CS menu items (e.g., Housing Transition/Navigation, Recuperative Care).
Skills:
Profinciency in EHR systems and data-driven decision-making.
Preferred Attributes Agility:
Ability to move to navigate the "start-up" nature of CalAIM implementation and evolving state mandates.
Advocacy:
A passion for serving the most vulnerable Californians and dismantling systemic barriers to care. •This is an in-person job, remote/virtual is not an option•
