Job Description
Job Summary Best Care Home Health is seeking a dedicated and highly skilled Designated Program Director to oversee and coordinate comprehensive programs (CFSS/245D) serving individuals with developmental disabilities in the home based setting. The ideal candidate will possess strong leadership capabilities, extensive experience in program development and a deep understanding of behavioral health and social work practices. As the Program Director, you will be responsible for ensuring program excellence, conducting assessments, developing person-centered support plans, compliance with regulatory standards, and fostering positive relationships with clients, families, staff, and community partners. Duties Conduct initial and ongoing assessments/evaluations for each client served. Ensure compliance with Minnesota Statutes and Standards in relation to delivery of services and documentation. Maintain confidential client records in accordance with HIPAA and state regulations. Coordinate services with case managers, client representatives/guardians, healthcare providers, interdisciplinary team professionals, and community resources. Participate in admission, discharge, and transition planning. Develop appropriate and effective person-centered support/care plans. Evaluate the effectiveness of service delivery, support/care plans and progress toward the client's outcomes based on measurable and observable criteria and update plans as needed. Respond timely and appropriately to client concerns, incidents, and crisis situations. Supervise, mentor, and support Direct Support Workers (DSWs) to include ensuring compliance with agency policies and procedures, providing additional training as needed and performing performance evaluations Promote a positive team environment focused on patient-centered care Perform additional duties as assigned by Clinical Director Education & Experience (must meet one) Bachelor's degree in a related field (e.g., psychology, social work, human services, or healthcare), OR Associate's degree + 2+ years of direct experience supporting individuals with significant behavioral or mental health needs, OR High school diploma/GED + 3+ years of progressive experience in behavioral, residential, or mental health settings
Additional Requirements:
Valid driver's license with a clean driving record Ability to pass MN DHS background study (including fingerprinting) Ability to complete and demonstrate competency in required

training Ability to work independently while maintaining alignment with team direction and supervisory guidance Strong interpersonal skills with the ability to build and maintain professional boundaries Excellent organizational skills and ability to manage competing priorities in a fast-paced environment Commitment to ethical practice, confidentiality, and person-centered care Proficient with basic technology (EHR systems, email, Microsoft Office) Proven management experience in overseeing programs serving individuals with developmental disabilities or related populations.. Experience working with diverse populations requiring individualized support plans is highly desirable.
