Licensed Professional -
Intensive In-Home Services Team Lead Reports To:
Clinical Director / Agency Director Position Summary The Licensed Professional - Intensive In-Home (IIH) Services Team Lead provides intensive, time-limited family preservation services designed to stabilize living arrangements, prevent out-of-home placements, and promote family reunification for youth up to age 20. Services are delivered primarily in the home and community through a team-based approach that addresses the clinical, behavioral, and environmental needs of the youth and family.
Minimum Qualifications:
Education, Licensure, and Experience:
Candidates must meet one of the following requirements: Hold a license, provisional license, certificate, registration, or permit issued by the governing board regulating a human services profession; OR Be a Registered Nurse licensed to practice in North Carolina by the North Carolina Board of Nursing with a minimum of four (4) years of full-time accumulated experience serving the
MH/DD/SAS
population.
Additional Requirements:
Must be at least 21 years of age. Ability to read, write, understand, and follow directions. Possess the education, competency, work experience, and skills necessary to effectively perform the duties of the position. Valid driver's license, reliable transportation, and current automobile insurance
Essential Duties and Responsibilities:
Clinical Leadership:
Serve as Team Lead for the Intensive In-Home Services Team. Provide or coordinate treatment services for youth and family members in collaboration with another licensed professional when appropriate. Coordinate and oversee the completion of initial assessments. Develop, monitor, and update Person-Centered Plans (PCPs). Participate in treatment team meetings and agency clinical staffing meetings. Provide supportive counseling based on the recipient's diagnostic and clinical needs. Monitor and manage presenting psychiatric, behavioral, and/or substance use symptoms.
Family and Community-Based Services:
Deliver services primarily in community settings, including homes, schools, shelters, libraries, and other approved locations. Assess and de-escalate family crises while implementing interventions to reduce the likelihood of recurrence. Provide parenting skills training and support to caregivers. Deliver self-help and independent living skills training to youth. Assist caregivers with implementing home-based behavioral support strategies. Ensure linkage to community resources and supportive services.
Care Coordination and Case Management:
Coordinate and monitor services on behalf of consumers and families. Act as an advocate, broker, coordinator, and monitor of service delivery systems. Facilitate movement across levels of care when clinically appropriate. Complete case management activities, including arranging, linking, monitoring, and integrating services and referrals. Ensure consumers are connected to the most clinically appropriate and effective services. Maintain ongoing communication with stakeholders, including DSS, DJJ, LME/MCOs, schools, healthcare providers, and other community partners.
Compliance and Quality Assurance:
Ensure compliance with all agency policies, procedures, and applicable state and federal regulations. Review and maintain complete, accurate, and compliant clinical records. Audit consumer charts to ensure documentation accuracy and regulatory compliance. Review progress notes and supporting documentation for completeness and timeliness. Ensure all required paperwork is submitted within established deadlines. Participate in quality assurance and quality improvement activities. Assist with policy implementation and program development initiatives.
Administrative and Supervisory Responsibilities:
Participate in staff interviews, hiring, and selection processes. Support staff development, training, and professional growth activities. Assist with program oversight and ensure departmental deadlines are met. Provide on-call first responder coverage on a rotating schedule.
Knowledge, Skills, and Abilities:
Strong clinical assessment and crisis intervention skills. Knowledge of family systems, behavioral health treatment, and community resources. Effective communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Strong organizational, documentation, and time management skills. Ability to maintain confidentiality and professional boundaries. Proficiency in electronic health records and documentation systems.
Working Conditions:
This position requires travel throughout the service area and the ability to provide services in home, school, and community settings. Evening, weekend, and on-call responsibilities may be required based on program needs.
Pay:
$60,000.00 - $70,000.00 per year
