Health Link Home Health and Hospice, a trusted Bay Area healthcare provider for nearly 20 years, is seeking a motivated and detail-oriented LVN Clinical Coordinator to join our Walnut Creek team. This essential on-site role supports the coordination of patient care, assists field clinicians, and helps ensure smooth communication between patients, families, physicians, and the clinical team. Medical, Dental, and Vision Insurance 401(k) with employer match Life Insurance Paid Time Off (up to 10 days annually) Supportive clinical and administrative team environment Meaningful work that positively impacts patients and families every day Key Responsibilities Coordinate and support care for home health patients Track, clarify, and input provider orders into the EMR system Communicate effectively with clinicians, physicians, patients, and families Organize and monitor medical supplies while maintaining OSHA standards Assist with onboarding and orientation of new staff Perform chart audits and administrative support tasks Support daily clinical operations within the office Qualifications Active California LVN License (Required) Minimum 1 year of nursing experience preferred Home health experience preferred Strong verbal and written communication skills Ability to manage high-volume phone communication professionally Excellent organizational and multitasking abilities Proficiency with Apple devices, EMR systems, and scheduling software Working knowledge of medical terminology Strong customer service and patient-care mindset BLS Certification preferred Work Location This is a fully on-site position based in our Walnut Creek office.
