Duties Remote Patient Care Coordinator (PCM) / Float Medical Assistant-Receptionist Position Summary The Remote Patient Care Coordinator (PCM) is responsible for providing ongoing patient support, care coordination, outreach, and documentation services for patients enrolled in the clinic's chronic and preventive care programs. This position is primarily remote but requires flexibility to travel to clinic locations and provide onsite support as a Medical Assistant (MA) or Receptionist when operational needs arise. Key Responsibilities Conduct patient outreach and monthly PCM care coordination activities. Document patient interactions and care plans in the electronic medical record (EMR). Assist patients with appointment scheduling, medication-related concerns, referrals, and care navigation. Monitor assigned patient caseload to ensure compliance with program requirements. Collaborate with providers and clinical staff to identify patient needs and barriers to care. Travel to clinic locations as needed to support staffing shortages, training initiatives, or operational demands. Perform Medical Assistant duties including rooming patients, obtaining vital signs, preparing charts, and assisting providers. Perform Reception duties including patient check-in/check-out, scheduling, insurance verification, and answering incoming calls. Maintain HIPAA compliance and protect patient confidentiality at all times. Complete other duties as assigned to support clinic operations. Qualifications High school diploma or equivalent required. Medical Assistant certification or relevant healthcare experience preferred. Experience in patient care coordination, medical office operations, or customer service preferred. Strong communication, organization, and documentation skills. Proficient with EMR systems and Microsoft Office applications. Reliable transportation and ability to travel between clinic locations as needed. Work Environment Primarily remote position. Must be available to travel and work onsite at clinic locations when requested. Occasional evening or adjusted schedules may be required to meet patient and operational needs. Perform patient intake procedures, including collecting medical histories, vital signs, and updating electronic health records (EHR systems) accurately. Assist physicians with dermatology, internal medicine, hospital medicine, urgent care, emergency medicine, and geriatrics cases by preparing patients and supporting clinical workflows. Support medication administration, basic life support (BLS), and basic physiology knowledge during patient visits or emergencies. Conduct clinical tasks such as X-ray imaging, vital sign monitoring, ICD coding for diagnoses, and basic anatomy assessments to facilitate accurate diagnosis and treatment plans. Coordinate patient care management by scheduling follow-up appointments, managing referrals within hospital or clinic settings, and ensuring timely communication between providers and patients. Maintain compliance with healthcare regulations by documenting procedures thoroughly and adhering to safety standards in outpatient or inpatient environments. Experience Proven experience working in hospital medicine, outpatient clinics, or emergency departments with exposure to Level I or Level II trauma centers preferred. Hands-on experience with EHR systems, ICD coding, medication administration, and vital signs collection required. Familiarity with dermatology procedures, X-ray operations, or Geriatrics is highly desirable. Prior roles involving medical-surgical units or internal medicine clinics will be advantageous. Demonstrated knowledge of anatomy and physiology principles along with strong understanding of medical terminology necessary for effective patient care coordination.

$18.00 - $21.00 per hour
