In this role, you will assess patient's understanding, needs and ability to engage in their own care outside of and in between provider visits. Your energetic approach will help coordinate care between multiple healthcare professionals, identify gaps in care, patient understanding or resources. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working in a fast-paced, supportive environment. Duties Reach out to patients via phone and in person to explain and enroll patients in CCM/RPM or other value based care programs. Touch base with patients on a regular basis to assess, educate and coordinate care or resources. Maintain accurate patient records, ensuring confidentiality and compliance with privacy regulations Communicate effectively with patients regarding appointment details, test results, and follow-up care Coordinate between patients and healthcare providers to ensure timely and quality intervention. Assist with managing medical terminology to accurately document patient interactions. Prior experience working in a medical office or healthcare setting is highly preferred Familiarity with medical terminology to ensure clear communication and accurate record-keeping. Office experience demonstrating organizational skills and attention to detail Excellent interpersonal skills with the ability to handle sensitive situations professionally
