Job Description
AuthoraCare Collective is currently seeking a Patient Care Coordinator. This is a Full-time hybrid position: Monday - Friday 8:00am - 5:00pm based at our Highpoint office. The Integrated Health coordinator works directly with Director and Senior Director of Home Health to provide high quality administrative support. Coordinator will assign and schedule home health visits (nursing, PT, OT, ST, MSW, HHA) using Homecare Homebase (HCHB). Ensure visits are scheduled within required timeframes according to the plan of care, patient availability, and clinician coverage areas. Monitor and adjust schedules daily to respond to cancellations, add-on visits, staffing changes, or emergencies. Optimize clinician productivity by managing visit clusters based on geography and discipline. Report missed visit report, LUPA report and productivity report daily and work to adjust schedule to meet organizational metrics. Coordinator will serve in an administrative role for the GUIDE program to include maintaining accurate and up-to-date patient records, caregiver, and service data within approved systems specific platforms. Ensure data completeness and accuracy with appropriate mailings and notification. Perform respite billing and communicate with partners and providers to ensure accurate and timely billing. Support compliance audits and quality reviews by ensuring timely access to clean, complete datasets. Communicate effectively both verbally and in writing with the GUIDE and Home Health Team, and community physicians. Identify patient needs in the home or facility and coordinate accordingly. Maintain accurate records in EMR (HCHB/Chartpath). Education and Experience :
One year's experience in a healthcare related role. Home Health experience preferred. Ability to establish and maintain effective working relationships with patients, other employees, and the public. Microsoft Word, Excel and HCHB experience strongly preferred. ChartPath experience is a plus.Key Responsibilities:
Administrative Support From general direction, provides clerical and general support and data entry for the GUIDE and Home Health service lines. Assist with incoming calls for GUIDE and Home Health (completes or directs them appropriately). Performs a variety of faxing and copying and mailings for GUIDE and Home Health and applicable programs. Clerical Support Performs other routine clerical tasks not directly related to above responsibilities, including using Microsoft programs to type and distribute correspondence, policies and forms as requested by Integrated Health leadership. Scheduling & Data Entry Responsible for entering referrals into Home Care Home Base. Responsible for ensuring referrals are compliant with CMS regulations. Responsible for scheduling visits for Home Health patients. Routine phone calls to patients/caregivers and clinical staff to finalize patient appointments. Add appointments for Home Health patients to schedules. Able to navigate or learn Home Care Home Base EMR and other applications. Consistently run and appropriately complete workflows as suggested by EMR. Patient Care Coordinator Must be customer service oriented, compassionate, and friendly. \ Strong decision-making skills and is a self-starter who can work effectively in a team environment. Maintain patient records usingEMR. HCHB
