This one is different. As a Patient Care Coordinator on our Chronic Care Management (CCM) team, you'll build real, ongoing relationships with the same patients — week after week, month after month. They'll call and ask for you by name. That's not an exaggeration. That's what our coordinators tell us. You'll work entirely by phone and EMR — no face-to-face patient interaction — supporting a chronic disease population that genuinely depends on your follow-through. You start hybrid — splitting time between home and our Tidwell or Magnolia clinic locations as needed. Prove your workflow and reliability, and the job becomes increasingly remote. WHAT YOU'LL DO
Ongoing Patient Relationships
Follow the same panel of patients over time — monitoring progress, identifying gaps, and proactively reaching out before small issues become big ones. Phone-Based Care Coordination
Outreach calls for scheduling, follow-ups, care plan check-ins, and chronic disease education. No in-person patient contact — your voice and documentation are your tools. Precision Documentation
Every interaction logged in eClinicalWorks with accuracy and consistency. In CCM, documentation isn't paperwork — it's patient care. Care Team Collaboration
Keep providers and clinical staff informed with organized, timely updates so nothing falls through the cracks. Scheduling & Coordination
Manage appointments, referrals, and follow-up workflows to close gaps in care. Occasional On-Site Presence
Minimal travel to our Tidwell or Magnolia locations as needed. Reliable transportation required.
WHAT WE'RE LOOKING FOR
The pay ceiling is $16/hr right now — we're not going to oversell it. What we can offer is flexibility, a team with real tenure, and a growth trajectory that's already in motion. To make the most of that, you'll need: Bilingual Spanish/English
Required. You'll communicate with our Spanish-speaking patient population every day. Certified Medical Assistant (CMA)
Valid MA certification required. Proven MA Experience
Clinical background in a primary care or community health setting. EMR Proficiency
Confident in eClinicalWorks or a comparable system. This role is documentation-heavy and pace-dependent. Attention to Detail
Missed follow-ups, charting errors, and scheduling gaps aren't recoverable in chronic care management. You catch things. Phone Presence
Warm, clear, and composed — especially with patients managing complex, long-term conditions. Self-Direction

COMPENSATION & SCHEDULE
Pay:
Up to $16.00/hr based on experience.
Work Setting:
Hybrid to start — flexible remote as performance and workflow are established.
Weekend Availability:
Must be open to one weekend day (Saturday or Sunday) with a 10-hour shift, based on assigned location.
Weekday Shifts:
12-hour shifts, Monday through Friday.
