Monday - Friday, with flexible hours as needed
Salary:
$23 - $27 per hour
Reports To:
Program Manager / ECM Supervisor Position Summary Zoya Enhanced Care Management (ECM) is seeking a compassionate and motivated Care Coordinator to provide comprehensive care coordination services to high-risk Medicaid Members. The ECM Care Coordinator will use a whole-person approach to assist Members in accessing medical, behavioral health, housing, and social services to improve their health outcomes and quality of life. This position involves field-based services, including home visits, community outreach, and collaboration with healthcare providers and community partners. Essential Duties and Responsibilities The ECM Care Coordinator will: Member Outreach and Engagement Conduct outreach to assigned Members through in-person visits, phone calls, and community outreach Establish trusting relationships with Members Educate members about ECM services and available resources Engage hard-to-reach and high-risk populations Assessment and Care Planning Complete comprehensive assessments of Member needs, including: Medical Mental health Substance use Housing Social determinants of health Develop individualized, person-centered care plans with Member input Review and update care plans regularly Care Coordination Coordinate services with: Primary care providers Mental health providers Substance use treatment providers Housing agencies Community organizations Schedule appointments and provide reminders Assist Members in accessing services Accompany Members to critical appointments when necessary Transitional Care Support Support Members during hospital admissions and discharges Coordinate follow-up care Ensure continuity of care Health Education and Support Educate members about: Medication adherence Chronic disease management Healthy lifestyle choices Help Members develop self-management skills Documentation Document all Member contacts and services accurately and timely Maintain complete and compliant Member records Use electronic care management systems Team Collaboration Participate in team meetings and case conferences Communicate regularly with supervisors and team members Participate in required trainings Required Qualifications Applicants must meet one or more of the following:
Education and Experience Preferred:
Bachelor's degree in: Nursing Social Work Public Health Psychology Human Services Certified Medical Assistant Or related field
OR Minimum:
High School Diploma or GED AND At least 2 years of experience working with high-risk populations
Experience Working With:
Medicaid population Homeless individuals Mental health Substance use disorder Chronic medical conditions Care coordination or case management Preferred Qualifications Highly preferred: Registered Nurse (RN) Licensed Social Worker Community Health Worker certification Care Coordination experience ECM experience Bilingual (Spanish, Somali, Amharic, Arabic, or other languages) Required Skills Must have: Strong communication skills Ability to work independently Ability to work in community settings Strong documentation skills Basic computer skills Reliable transportation Valid driver's license Work Environment This position is: Field-based Includes home visits Includes community visits Includes office work Physical Requirements Must be able to: Travel to client locations Sit, stand, and walk for extended periods Background Requirements Must pass: Criminal background check Driver's license check Benefits Zoya Enhanced Care Management (ECM) Competitive salary Flexible schedule Paid training Career growth opportunities Supportive team environment Why Join Zoya Enhanced Care Management (ECM)?
