Patient Care & Operations Coordinator

Sage & Stone Wellness is a rapidly growing telehealth and wellness practice specializing in weight management, hormone optimization, wellness medicine, peptide therapy, and concierge-style patient care. We are looking for a highly motivated, detail-oriented team member who thrives in a fast-paced environment and wants to grow WITH a business — not simply work for one. What We're Looking For We are seeking someone with a medical background such as:

• Medical Assistant (MA)
• Certified Nursing Assistant (CNA)
• Licensed Practical Nurse (LPN)
• Patient Care Coordinator

Responsibilities Responsibilities may include:

• Patient scheduling and appointment coordination
• Following up with patients regarding care plans and appointments
• Medication order preparation and coordination
• Assisting with EMR/chart workflow
• Patient communication and customer service
• Managing daily operational workflows
• Supporting provider efficiency and organization
• Helping generate marketing ideas and growth initiatives
• Assisting with local outreach and business development opportunities
• Identifying workflow inefficiencies and helping improve processes Position Structure
• Hybrid role with flexibility
• Approximately 2-3 days per week in-person in the Cumming, GA area
• Remaining work may be completed remotely depending on workload and responsibilities

• Medical office or patient care experience
• Strong communication skills
• Experience with scheduling systems or EMRs preferred
• Social media, marketing, or business growth interest is a plus
