The Population Health Coordinator is responsible for the completion of set processes and protocols. Supports the teams in meeting financial, clinical and service goals.Schedule :
This position isFT, 38.75
hours per week, Monday- Friday. Employees are required to have flexibility to work any of our 7.75-hour shift schedules during our normal business hours of 8am
- 4:30pm EST. The schedule will be determined by the hiring manager upon hire. This is a hybrid role
- once training is completed, you will be working 2 days onsite and 3 days remotely. You will be required to complete training at 5 Neponset St Worcester, MA. Training will also be hybrid
- training schedule will be determined upon hire. If you reside in Leominster, MA , you'll enjoy the flexibility to telecommute

225 New Lancaster Rd, Leominster, MA 01453Primary Responsibilities:
Contacts patients for the purpose of achieving established outreach program goals. This may include contacting patients via telephone, email and/or direct mailings to stress the clinical importance of receiving their recommended lab work and attending their medical appointments At the request of a Clinician, may place lab order and/or schedule appointments in electronic medical record system Generates reports using Healthy Planet Registry in Epic EMR to identify patients who require additional labs to be ordered as well as identifying patients who are over-due for labs Consults with clinicians to determine required population reports and/or patient lists for analysis and outreach Communicates and collaborates with care coordination staff to optimize patient care, and supports patient care plans by assisting with linkages to, coordination of, and monitoring identified service needs Recognizes and reports data inconsistencies, gaps in performance, and opportunities to improve results to appropriate personnel Performs other duties as assigned What are the reasons to consider working for UnitedHealth Group? High School Diploma/GED (or higher ) 1+ years of experience in a customer service or healthcare related field Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI) Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service Ability to complete training at our 5 Neponset St Worcester, MA office Ability to work onsite for 2 days and 3 days remotely (once training is completed) Ability to work full-time, Monday- Friday. Employees are required to have flexibility to work any of our 7.75-hour shift schedules during our normal business hours of 8am

Preferred Qualifications:
Microsoft Office Suite Proficient with data manipulation including the ability to develop spreadsheets, build data tables and extract data Proficiency with electronic medical record systems Knowledge and understanding of medical terminologySoft Skills:
Ability to work independently and as a team, and maintain good judgment and accountability Demonstrated ability to work well with health care providers Strong organizational and time management skills Ability to multi-task and prioritize tasks to meet all deadlines Ability to work well under pressure in a fast-paced environment Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others- All Telecommuters will be required to adhere to UnitedHealth Group's Telecommuter Policy.
