About Us:
HOPE Community Medicine is a mission driven Federally Qualified Health Center (FQHC) dedicated to providing high-quality, affordable healthcare to the medically underserved communities of Deep East Texas. Summary:
The Referral Specialist is responsible for managing and processing patient referrals for HOPE Community Medicine. Working under the supervision of the Referral Coordinator, this role ensures referral accuracy, compliance, and timely communication between providers and patients.
Expectations:
Ensure accurate and timely processing of patient referrals. Maintain compliance with referral documentation requirements. Provide professional communication with patients, providers, and outside facilities. Abide by Rules of Confidentiality and follow organizational policies. Demonstrate awareness of and compliance with the mission of HOPE Community Medicine.
Job Duties:
Manage and process up to 100 patient referrals for HOPE Community Medicine. Track referral status and communicate updates to providers and patients. Collaborate with the Referral Coordinator/Director for quality assurance and workflow improvements. Maintain referral records in compliance with HIPAA and regulatory standards. Assist with referral-related reporting and audits as assigned.
Education:
High school diploma or equivalent required; associate degree or healthcare-related coursework preferred.
Skills:
Experience with medical referrals strongly preferred. Strong organizational and communication skills. Ability to manage high-volume workload in a fast-paced environment. Proficiency with electronic health record systems preferred.
Physical Requirements:
Must be able to remain in a stationary position for extended periods. Ability to move about inside office or clinic sites as needed. Occasionally required to lift up to 20 pounds. Ability to communicate effectively in person and over the phone.
