Job Description

Outreach Coordinator - Home Health | Savannah River Site Region About the Role We are looking for a motivated and relationship-driven Outreach Coordinator to grow our presence and expand access to care for patients in the Savannah River Site region. This is a boots-on-the-ground role built for someone who knows the local healthcare landscape, has earned the trust of physicians and care teams, and understands the unique needs of the community we serve. Responsibilities Develop and execute targeted outreach strategies to promote home health services across healthcare networks and communities connected to the Savannah River Site region Build and maintain strong, trust-based relationships with physicians, hospitals, clinics, social workers, case managers, and other referral sources through consistent, meaningful engagement Educate potential referral partners on our home health programs, eligibility criteria, and the specialized care we provide to DOE-related patients — clearly communicating our mission and value Manage a portfolio of key accounts, providing responsive service, addressing questions, and ensuring referral sources have what they need to confidently send patients our way Identify and pursue new partnership opportunities with healthcare providers, facilities, and organizations serving the SRS-impacted population Represent Giving Home Health Care at community events, health fairs, and informational sessions relevant to nuclear workers, their families, and the providers who care for them Track outreach activity, referral data, and pipeline progress — using that information to sharpen strategy and report on results Qualifications Experience in home health, hospice, or medical sales is strongly preferred — candidates with a background in adjacent healthcare sales fields and a demonstrated ability to build referral relationships will be considered Existing relationships with physicians, specialists, or care teams in the Savannah River Site region (South Carolina, Georgia, and surrounding areas) are a significant advantage Familiarity with the SRS community — including the workers, retirees, families, and healthcare providers connected to the site — is highly valued Strong communicator, both one-on-one and in group settings, with the ability to speak confidently about healthcare services and patient eligibility Self-starter who can manage a territory independently, prioritize relationships, and stay organized across multiple accounts Genuine passion for serving patients who have given their careers to the nation's nuclear mission Reliable transportation and the ability to travel locally and regionally on a regular basis Proficiency in Microsoft Office (Outlook, Word, Excel); CRM experience a plus Associate's degree or higher preferred; relevant experience will be considered in lieu of formal education
Pay:
$24.00 - $30.00 per hour
