Job Description

Black Hills Special Services Cooperative is seeking a compassionate and organized RN/Medical Services Coordinator to lead health support services for individuals with developmental disabilities. This rewarding role combines nursing leadership with meaningful, person-centered care by supervising and supporting Case Nurses and Medical Assistants while working directly with the people we support to help them access the healthcare they need. You'll coordinate medical services, oversee medication management, maintain medical records, collaborate with healthcare providers, and advocate for individuals to ensure their health needs and personal goals are met.   • Provide supervision, training and support for the Case Nurse ( s ) and medical assistants .
Provide Medication Aide training and recertification training for DD staff. Conduct evaluations with Case Nurse ( s ) and M edical A ssis t ants. Assign caseloads and have their own caseload of people supported. Maintain a high level of awareness of the health and medical goals and needs of each person served and ensure that routine and emergency medical and health needs are met. Aid , support and encouragement to people supported in reaching their health goals. Coordinate healthcare services and work directly with people supported to ensure access to medical appointments, treatments, medications, and other healthcare resources necessary to meet their individual needs. Maintain current medical files, as dictated by licensing standards and ensure that documentation is current . Maintain current immunization records. Make sure people supported always have an adequate supply of medication . F ollow-up on recommendations and orders from medical professionals. Participate in nursing on-call rotation.   • Current valid South Dakota RN nursing license Current valid drivers' license Ability to pass criminal records check, background check and pre-employment drug screening Organizational, leadership and communication skills and the ability to self-motivate Benefits
